How We Use Your Information

Our opening-interest form is hosted by Microsoft Forms. It collects the contact details you provide, your preferred contact method and your permission to receive practice opening and scheduling updates.

Smile Restoration Specialists uses those responses to manage the interest list and contact you about opening and scheduling. Responses are stored in Microsoft Forms. An automated notification containing your submitted answers is also sent to our practice interest inbox and distributed to authorized practice team members through Microsoft 365.

The form is not intended to collect symptoms, medical history, insurance details or patient records. Signing up does not establish a patient relationship, reserve an appointment or request treatment.

To withdraw permission to contact you or request removal of your interest-list information, contact interest@smilerestorationwi.com. Please do not include medical or dental information in your request.
